Medical Records
Copies of your medical records for services performed
at Mercy may be obtained by contacting our Health Information Management
Department. Requests can be made in person or in writing by utilizing
the authorization
for release of information form. Please be sure to include your full
name, other possible names used, birth date, social security number,
phone number, type of services received and date of service, as well as
a witness signature.

Fees for providing copies are $1.00 for the first
page, and 50 cents for each additional page. Allow 10 working days for
requests to be filled. If you have questions about the release of
medical information, call the Health Information Management office.
Due to federal privacy laws regarding the release of
medical information, we are not able to answer requests for medical records
made through the internet.
